SurroPlans Passport service

The first information needed via email

Please, remember to always write to passport@surroplans.com CCing your SurroPlans advisor for all things related to the passport

Please, prepare and send us these documents and information:

Birth certificate of the baby - a scanned copy or a photo (for each baby)

Baby's eye color, hair color, height (for each baby)

Copy of passports - a scanned copy or a photo (for each parent)

Your home country permanent address

Your SurroPlans phone number (provided to you by SurroPlans as part of your Privacy Shield - ID card)

The email addresses you would like to use (if the ones copied to this email are not the right ones)

Your temporary address in the US (this is where the passport will be mailed so please make sure you have access to the mail box)

Copy of the flight itinerary back home issued with the baby/ babies name on it (mandatory only if you chose the Premium Expedite service or for in person application) (for each baby)

How to make baby's passport picture

Once you have shared the information above (the most urgent information), we will need to get passport- approved baby pictures (for each baby)

Please take a few pictures/headshots of your baby with your smartphone/camera with all head and half chest.

Make sure there are no shadows on your baby or toddler's face.

It is better if baby's eyes are open but it is OK if they are not.

Send the pictures to passport@surroplans.com CCing your SurroPlans advisor. Our team will check the pictures and remove background.

Once ready, we will email them back to you and you will be able to have them printed:

What will happen next

If you opt for an in-person appointment

Once we have received your first email with the information above, we will complete three forms for you and we will send them back to you by email.

If you want to receive the passport at your selected address

We will send you:

DS-11 form — once you receive it, please check if all the information is correct. You will have to have this form printed single sided. DO NOT SIGN IT. You will have to sign it during the ID validation appointment.

Letter of authorization — you will receive it pre-filled in by us, by email. You will need to print it, sign it, add your full names and then "Father" or "Mother". These 3 elements will need to be handwritten. See example below:

Statement letter for no SSN — you will receive it pre-filled in by us, by email. You will need to print it, sign it, add your full names and then "Father" or "Mother". These 3 elements will need to be handwritten. See example below:

We will secure an appointment for you at your closest acceptance facility for ID and documents verification.

You need:

Print documents — Once you receive these documents, as mentioned above, you will need to print them out for a reminder: DS-11 form, Letter of authorization and Statement letter for no SSN. Please let us know if you do not have access to a printer and we can help you find the closest place to your temporary address where you can have these printed single sided.

Add your name(s) and sign both Letter of authorization and Statement letter for no SSN. DO NOT sign the DS-11.

Please, remember you will all need to be present physically (parents and baby)

Things you will need to have with you at your appointment for ID validation

Cash — You will need to have issued a money order during your appointment to pay for the passport fee. We will book the appointment at a facility where you can have your money order issued otherwise we will give you instructions on how to get it. In order to pay for the money order, please bring cash. Amounts you will need to pay (so make sure you will have this in cash):

- If you have the Basic Expedite service: 35$ + 160$ (total 195$)

- If you have the Premium Expedite service: 35$ + 181.36$ (total 216.36$)

Approved and printed baby passport pictures

1 original copy of the baby birth certificate (Apostilled not necessary - it will be returned along with the passport)

A copy of the baby birth certificate

Parent's passports

1 copy of each parent's passport, front and back

Letter of authorization (Printed and signed - see above)

- The original signed copy will be added to the sealed envelope by the officer

- Make sure you scan it before you go to the appointment and send a scanned copy (with signature), handwritten names and "father/mother" to passport@surroplans.com before you add the original to the envelop

Statement letter for no SSN (Printed and signed - see above)

- The original signed copy will be added to the sealed envelope by the officer

DS-11 form (printed 1 sided, in color, NOT signed - see above)

Baby's itinerary flight ticket (The document needs to show the flight number and ticket or reservation)

What will happen at the appointment

The post office will:

Review and take all the documents and pictures

Make you sign the DS-11 form under oath

Put it all in a sealed envelope

You will:

Take this sealed envelope to a UPS store (On the label there will be mentioned the closest UPS store to your local address in the US)

Request a (free) express envelope and apply the prepaid UPS label we sent you

Basic Expedite service

If you choose the Basic Expedite service, the passport will be sent to you either via Fedex or UPS at a local CVS/Walgreens location for pick-up.

Premium Expedite service

If you choose the Premium Expedite service, the passport will be mailed back directly from the passport bureau via USPS priority mail.
